The moment Dean said Purgatory is the moment Sam’s world shattered, again. and all the guilt came rushing through, all the pain, all the things he wish he could’ve done but he couldn’t. He probably thinks ‘how could I not figure this out’, ‘How could I left him there’ and all reason is gone, it doesn’t matter that he couldn’t, that he had no way to figure this out, to Sam, it matters only that he failed to save Dean. Again.
